Agape of the Ozarks, LLC provides Consumer Directed Services (CDS). CDS is a Medicaid based program which provides services to individuals with disabilities similar to in home care, enabling them to live independently.

Consumers in the CDS program are trained to hire their own attendants to assist them with daily tasks.

The CDS Program is designed to assist individuals with disabilities to receive the attendant services they need to live safely in a home setting of their choice, instead of living in a more restrictive institutional environment.

Agape of the Ozarks, LLC is a consumer-managed program. One main component in the CDS program is that the individual who receives CDS services is the employer of the attendant. Agape of the Ozarks, LLC consumers are responsible for locating, hiring, setting work schedules and managing their own attendants. A qualified friend or family member may be hired as an attendant, with the exception of a spouse, legal guardian or anyone under the age of 18.

To qualify for consumer-directed services, a person must be:

  • Medicaid eligible (MO HealthNet eligible)

  • 18 years of age or older

  • Able to direct their own care

  • Capable of living independently with the services of CDS

  • Require at least a nursing facility level of care.

Individuals on Medicaid will need to meet Missouri Division of Senior and Disability Services (DSDS) requirements. DSDS will complete an assessment to determine the individual’s personal care and home care needs (bathing, housekeeping, transportation, laundry and other needs).

A personalized care plan will be forwarded to Agape of the Ozarks, LLC. Once a care plan is received, Agape of the Ozarks, LLC will help you hire the appropriate aide.

Agape of the Ozarks, LLC manages on your behalf:

  • Criminal and abuse/neglect background checks on attendant candidates

  • Processing time clock entries and payroll for your attendant(s)

  • Assistance in completing necessary forms

Meet the owner

Ashanti Tate began working with Medicaid funded agencies beginning at age 19. She completed her undergraduate degree at Wichita State University where she majored in Social Work. She also earned a Masters of Social Work and Masters of Instructional Leadership in Education from the University of Illinois at Chicago.  


In her most recent role up until August of 2024, she served in leadership at Heartland Center for Behavioral Change- a nonprofit organization that helps people begin a new life – free of alcohol and drugs. In this role she provided oversight to counselors, case managers, peer supports, and medical staff in an effort to beat the opioid epidemic. She is also a state recognized trainer on, “How to Launch DEI in Non-Profits” and “How to Beat Burnout-Post Pandemic.”

Tate believes she was created for two sole purposes: to serve God and serve His people. She exercises her beliefs by mentoring at-risk youth thru Big Brothers Big Sisters organization, serving as a foster parent to at-risk minority children, hosting empowerment brunches for women, and also through her sorority, Sigma Gamma Rho, Inc. Tate also serves on the board for the Missouri Foundation for Health. 

In 2020 she was selected as one of the top 40 professionals under the age 40 by Springfield’s Business Journal. In 2021 she was voted the best dressed female by 417 Magazine. In 2024 she was selected as one of the 20 Most Influential Women by Springfield’s Business Journal. 

Her most honorable roles are being a mother to her three children: Zion, Emmanuel, and Myracle; and a wife to her husband, Pastor Terrence Tate.
